All personnel working within the NSLS complex are required to follow the requirements set forth in the BNL Subject Area, Lockout/Tagout (LOTO).
Additional NSLS specific LOTO requirements follow. These requirements apply to all except for BNL Plant Engineering personnel:
- All NSLS LOTO operations that require a red hold tag shall be logged in the LOTO logbook located in the NSLS Control
Room. (Note: Lock only applications are not required to be logged)
- Group LOTO: Only the Responsible Employee attaches a lock AND tag. Authorized Employees may attach additional locks with no tags.
- Single LOTO: An Authorized Employee may attach a lock with no tag when working on a system with a single disconnecting means if that disconnect always remains in sight and the task is completed within one work shift. The lock must have employee identifying information (a name or picture or life number).
A tag is not required under the following circumstances:
- All SDL LOTO operations that require a red hold tag shall be logged in the LOTO logbook located at the SDL Control Console. (Note: Lock only applications are not required to be logged.) Tag use requirements are the same as listed for the NSLS.
- All entries shall be in the following format: Tag #, Date Out, Location, Lock #, User, Date Returned.
- Long-term NSLS lock out tag outs shall be maintained in the NSLS Control Room LOTO Database. This database shall contain information about
systems that are required to be locked out for greater than one year. The status of these systems shall be reviewed annually, and results of the
review shall be logged in the database. The focus of the review should include at least the following:
- Check and document the legibility of the red tag
- Document the reason to continue with the long-term LOTO
- Performing electrical LOTO zero energy verification measurements requires personnel to, “Work on or Near Energized Conductors.” This work must meet the requirements contained in NSLS ESH PRM 1.5.0.
