BLOSA Revisions

Please keep your beamline's BLOSA form up to date by either incorporating the changes shown below, or starting from a new template file.

Review the Training Guidance for specifics about what you should be telling users.

DATE DESCRIPTION of CHANGE
10-25-04 Added important bullet for "Configuration Control". See new text in blue in the template file and modify your existing BLOSA form with the change. Remember to update the Revision Date at the top of your form!
10-1-03 Updated the top section relating to new TLD policy for short-term users.

INSTRUCTIONS:

  • Please review the template file and incorporate the BLUE TEXT into your current BLOSA form and delete anything that's crossed out. Customize it for your beamline.

NOTE : The blue text and crossed-out text are only there so you can see what the changes were; on your own form, you should change the blue to normal black text, and delete any crossed-out text.
6-1-03 TEMPLATE: All beamlines must use this new template - get rid of your old form, and use this file as the starting point. Make sure you at least address the text in red in your BLOSA form.
1-22-03 Updated the top section "Upon Arrival"
2-25-02 Changed "Beryllium handling" to "Beryllium handling and damage cleanup"
